Duties & Responsibilities
Under the administrative and clinical supervision of the Clinical Services Manager, and in accordance with company policy, goals, and purposes, the Case Manager is responsible for developing and implementing an Individual Program Plan for each assigned resident in accordance with program policy. The Case Manager is responsible for providing case management services and monitoring progress throughout treatment for assigned residents. The Case Manager is responsible for ensuring that each resident receives maximum benefit from treatment. The Case Manager is responsible for suggesting and recommending changes in program, policy and procedure. The Case Manager is responsible for providing high quality services under the lease restrictive conditions feasible. Must know and understand the needs of chemically dependent people in general, as well as the effects of co-morbid disorders on the recovery process. The Case Manager is accountable for the whereabouts of residents in care; assists in meeting health and hygiene needs; maintains and re-establishes group and individual control, when needed; participates in and models meaningful social experiences; prepares legal and other documents in accordance with program policy; transports residents in accordance with program policy, as necessary; serves as a liaison with community resources, in accordance with program policy.
Education & Experience Required
Must be registered with the Washington State Department of Health as a counselor, and must be a Chemical Dependency Professional or Chemical Dependency Professional Trainee. Must have acceptable experience in human services, especially in writing reports and recording resident observations. Must have a valid Washington State Driver’s License with a driving record acceptable to the company’s insurance carrier. Additional experience desired: certification in alcohol/addiction studies; experience in residential treatment, and/or working with individuals with co-morbid disorders; advanced training in group process or educational programming design.
